In a medium saucepan, whisk together the heavy cream, whole milk, granulated sugar, cocoa powder, and malted milk powder over medium heat until well combined. Heat until the mixture is hot but not boiling, stirring occasionally.

In a separate medium bowl, whisk the egg yolks until smooth and slightly pale. Slowly pour a ladle of the hot cream mixture into the yolks, whisking constantly to temper them. Repeat with another ladle to prevent the eggs from curdling.

Pour the tempered egg mixture back into the saucepan. Cook over medium heat, stirring constantly with a wooden spoon, until the mixture thickens slightly and coats the back of the spoon, about 5-7 minutes. Do not let it boil.

Remove from heat and stir in the vanilla extract. Pour the mixture through a fine-mesh sieve into a clean bowl to remove any lumps. Cover the surface with plastic wrap so it touches the custard directly (to prevent a skin from forming) and refrigerate until completely chilled, at least 4 hours or overnight.
